We can assist one time, or occasionally as needed. For example, our loved one may need help when being discharged from the hospital. When called on, we will coordinate pick up with a discharge planner, a social worker, or the nursing staff at a hospital or rehab facility. Our caregiver will follow the patient-specific plan, pick up prescriptions, and get your loved one comfortably settled in at their home, or other living facility.

We can make phone calls to the family with progress reports and updates as requested or needed. Working as a team, we can ensure adherence to the discharge plan while keeping the client comfortable, thereby reducing or eliminating the need for readmission.
